For most hospital referrals you can now choose the time and place where you would like to be seen because we use the "Choose and Book" software standard.
When you collect your prescription (if you are on regular long term treatment), you will also receive a tear-off slip listing your name and computer number, your repeat medications, and the planned review date for these medications.
A repeat of your prescription may be obtained by any of the methods listed below. Except in emergencies, the prescription will be ready for collection from reception after 1pm the day following the request being made. It is possible to have your prescription sent direct to some of the Chester-le-Street pharmacies, but next day availability can not then be guaranteed.
Telephone (0191) 387 1759 between 11am and 4pm, quoting your name and computer number (on the top right hand of the repeat prescription list).
Leave your ticked repeat prescription slip in one of the two collection boxes, one is situated just outside the surgery main door for out of hours use, and the other is just to the left of the main reception counter.
Send a stamped addressed envelope with your ticked repeat slip to the Health Centre. We will post your prescription to you the next day. Remember to make allowances for postal delays/Bank Holidays etc.
Prescriptions for up to 3 consecutive months in advance can be requested, with your doctor’s agreement. Normally your doctor will need to review your treatment with you at least annually.
Please order only the medications that you need, by ticking the box to the right of that medication. Please do not order any medications that you do not need.
