Before looking at any consultation document and using the prompts provided on the separate sheet, start by thinking what you would want to say about:
  (a) the Regulations to govern the use of the Register of Information
  (b) the sort of information that you think should be recorded from now onwards about donors:
  (c) donor anonymity in general

2. mark your list according to which you think should have highest priority - i.e. which do you care about most strongly at this moment in time (1 being the highest priority). Note why these are the most important points for you.

3. Look at the list below and see if you want to add or alter anything to your answers to Question 1 when you think about your responses as, say, a parent and a daughter; or a donor offspring and a brother etc.
    For parents of donor offspring: as a parent as an individual as a partner (if in an adult relationship) as a son/daughter/family member of the wider family
    For donor offspring as a person conceived through donor assisted conception. as a son/daughter as a brother or sister of someone conceived through donor assisted conception as a brother or sister of someone conceived another way
    For donors as a donor as a partner (if in a relationship) as a parent of non donor children, if a parent as a son/daughter/family member
    For others as a family member of someone conceived using donated sperm or egg.

In doing this, you may want to think about the following:

  1. in relation to access to information, what helps me and makes it easier in managing this part of myself?
  2. and what hinders me/gets in the way/makes it more difficult in managing this part of myself?)

Some matters to consider as you put together your response:
  Would you like the Regulations to allow, or make easier, any of the following?
 
Access to past information about the donor

- biographical
- medical
- information about his/her wider family
- letter from donor
- photo of donor
- written record - HFEA form and/or medical notes

Access to current information about the donor

- medical
- lifestyle/biographical
- number of other children conceived through whatever route
- information about his/her wider family

Chance to meet with donor face to face: - for donor offspring only
- for parent/s only
Knowing the identity of the donor - for donor offspring only
  - for parent/s only
  Should there be any changes to the way in which the birth or medical records of donor offspring are recorded which ensures that they will find out about the nature of their conception regardless of whether their parents or others tell them (as with adopted people or people born as a result of surrogacy)?
  Should medical records automatically include the fact that the conception was donor assisted?
  Which public bodies or professional groups, if any, should have automatic right of access to information (e.g. health, insurance, employers)?
