To help you, you can download this form from here and send that in to the Elections Office at Stratford-on-Avon District Council, Elizabeth House, Stratford-upon-Avon, Warwickshire CV37 6HX
As the form is a legal document it needs to be printed, signed and sent to your local council. If you wish to complete a form but are unable to print it out, you can call the Stratford Tory office on 01789-292723 who will send you a form in the post. Please note that you need to get your form into your local council at least six working days before an election
